The latter is the case for Banana Republic, where sales associates are trained to measure the garment to your specifications, and then send the clothing out to be altered. Turnaround time for this is usually three to five days, so a lot of the time, this is an easier option if you don't have a tailor.
Other stores such as Nordstrom or Macy's offer the service, with varying requirements for obtaining the service. Inquire with a department or store manager to find out the specifics at your local store. Keep in mind, in every case, the clothing should be purchased at said store.
